Plenty of trees get removed that did not need to come down, and a few get left standing that should have come down a year ago. We look at the thing in front of us: the canopy, the trunk, the root flare, the soil around it: and tell you which one you have.
The useful question is rarely whether a tree is sick. It is whether it is dangerous, whether it is getting worse, and whether anything can be done about it: and those three have different answers. A tree can be in visible decline and pose no risk at all. A tree can look fine and be structurally compromised. Sorting that out is what an assessment is for.

We start at the ground. The root flare should be visible where the trunk widens into the soil, and on a surprising number of declining trees it is not: buried under fill from a construction project, under years of accumulated mulch piled against the trunk, or under a raised bed somebody built around it. A buried flare stays damp, decays, and kills mature trees slowly enough that nobody connects the cause to the effect. Girdling roots that have wrapped the trunk do something similar.
Then the trunk and the unions. Vertical seams, sunken or discoloured areas, bark coming away in patches, and any fungal fruiting body: mushrooms or shelf fungus at the base, on the trunk, or over the root zone: are all worth taking seriously. Fungus at the base in particular usually indicates decay that started well before it became visible, because the fruiting body is the last stage rather than the first.
Recent site work explains a large share of the decline we get called out to. Trenching for a line, a driveway extension, fill brought in to raise a grade, heavy equipment parked over a root zone: any of these can sever or suffocate a significant fraction of a root system, and the tree often does not show it for two or three seasons. If something changed near the tree in the last few years, that is usually where the answer is.
